The state Department of Agriculture conducts annual inspections of food-serving businesses to ensure public safety and safe food-handling practices. Schools are inspected twice per year, according to the Bureau of Food Safety and Laboratories.

Whether an establishment is considered compliant or non-compliant is the discretion of the inspector. There are, however, several violations for which inspectors should automatically consider establishments non-compliant. Called "critical violations," they include food temperature issues, employee hygiene and issues with chemicals and how they're handled.

Depending on the severity of the situation, inspectors and their supervisors could file citations or close an establishment.

Below is a list of establishments that were inspected April 18-24.

OUT OF COMPLIANCE

Regular inspections:

Ma & Pa Pizza, 5850 Delta Road, Delta, 9 violations

Action: Violations included food employee observed without proper hair restraints; employee observed touching ready-to-eat foods with bare hands; raw eggs stored above ready-to-eat food in walk-in refrigerator; in-use plastic containers chipper and cracked; clean containers stored inside soiled containers; inside shoot of ice machine observed with white slimy film; clean food equipment stored in wet manner; flooring around pizza prep area cracked and not easily cleanable; shelving in the kitchen extremely dusty; and hood vents heavily soiled with grease drippage and black soot.

Note: This is the third time this restaurant has been ruled out of compliance since April 2015.

Lewisberry Convenience, 658 Wyndamere Road, Lewisberry, 21 violations

Action: Violations included cheese and hot dogs not properly date marked; gas can and hand soap stored on same shelf as frozen drink base; glass cleaner squeeze bottle hung directly over cutting board and pizza trays; original food certificate not posted in public view; employee observed wearing single-use gloves without prior hand wash; food employees observed without proper hair restraints; raw eggs stored above milk in walk-in cooler; pizza paddles stored on top of dirty oven; case of eggs stored directly on floor; beef patties held at improper temperature; no ingredients sign displayed in self-service donut case; cutting board observed deeply scored; sanitizer observed at improper concentration levels; inside of microwave observed with food residue; accumulation of black mold-like residue observed on ice machine; accumulations of dried food debris observed on and around dispensing nozzles of cappuccino machine; accumulation of white mold-like residue observed on soda nozzles; accumulation of food debris and black mold-like residue observed on shake machine; accumulation of dirt on walls and floors; single-use cups stored directly on floor; several lights not shielded or shatter proof; several ceiling tiles observed with water damage; and several holes observed in bathroom.

Lyndon Diner, 1353 Kenneth Road, 9 violations

Action: Violations included in-use tongs stored on oven door handle; wiping cloths stored in unclean condition; time used in lieu of temperature to control ready-to-eat foods; interior surface of pancake cooling equipment cracked and chipped; shake machine observed with food residue; table-mounted can opener observed with food residue; table under grill observed heavily encrusted with grease and soil accumulation; plumbing system not maintained in good repair; paper towels sitting directly on wet counter; and paper towel dispenser empty at hand wash sink.

Note: This is the fourth time this restaurant has been ruled out of compliance since January 2016.

Kokomo's Sports Bar & Grill, 691 Yorktown Road, Lewisberry, 14 violations

Action: Violations included food not being date marked; container of bleach stored above dish machine; in-use tongs stored on oven handles; time used in lieu of temperature to control ready-to-eat foods; interior surface of chest freezer observed cracked; cutting boards observed deeply scored; sanitizer observed at improper levels; deli slicer blade observed with food residue; inside of microwave observed with food residue; black mold-like residue observed on ice machine; table-mounted can opener observed with food residue; accumulation of food debris observed on walls and floors; no hand wash sink located at bar; screen door observed with hap; several holes observed in wall; and accumulation of static dust observed on ceiling.

Note: This is the second time this restaurant has been ruled out of compliance since March 2016.

Meadow Hill Family Restaurant, 2935 E. Prospect Road, 12 violations

Action: Violations included employee's open beverage container observed in food prep area; cigar found in kitchen area; plate being used as scoop in cooked sliced potatoes; wet wiping cloths not stored in sanitizer solution; bags of flour and potatoes stored directly on floor; chicken and beef stored directly on floor; desserts stored at improper temperature; loose rubber gaskets observed on freezing units; ice machine observed with rust-like residue; milk shake machine observed with food residue; clean dish racks stored on floor; utensils stored in cardboard-lined drawer; hand wash sink blocked by bus cart; mold-like substance and static dust observed on fan guard; and mops not being hung to air dry.
